It is not simple, but it is both a mistake and institutional bias. Neither are unique to FN of course, but they are more prone to it (you could make a nice coffee table book of color photos of mistaken and misleading graphics from Fox News.
This is best thought of as neither bias nor harmless mistake, but shoddy journalism. It is human nature to want to perceive and understand stimuli in ways consistent with existing schemas. Professional journalism should employ processes and practices to check those tendencies.when they consistently don't, it is because they are bad journalists.
