As it happens I'm watching an episode of The Simpsons each day on DVD, both
the episode and the audio commentary. I'm halfway through the first season
and the creators are pointing out the same issues in continuity as they
watch the episode 10 years after it first aired. One issue that keeps
recurring is they sent the first batch of episodes to Korea to be animated
before the first episode aired so they had no feedback on how the earliest
episodes were received or if they had to even worry about thinking past a
first season. They didn't even bring in a production designer to solidify
the design until halfway through the season and they didn't bother with a
floor plan for the house, school, or nuclear plant until later in the
season.
