I have found Universe upgrades to be extremely clean. We upgrade our customers on a consistent basis and I've never had a real problem because of it. Now application upgrades those can be a little dicier ;)

Does not mean you do every upgrade, but it does requrie an effort to stay fairly current. With that many users, you probably have a test environment where upgrades could be evaluated prior to rollout to the live system.
