From that very document: Due to lack of a single specification, there are considerable differences among implementations. Implementors should "be conservative in what you do, be liberal in what you accept from others"...
That said, I did speak too soon. The convention I referred to is for use within Excel and doesn't work for CSV. I have a bit more helpful related info in response to Mark's inquiry.
