Might be worth taking a stroll through the archives; Glenn Herbert wrote on the 
mechanisms used when files are reshaping themselves (which might include 
extending into overflow, creating an oversized record, or dynamic file 
split/merge activity.  Each of these takes a latch on the file header ("group 
#0") and is the principal cause of a self-deadlatch situation.  Another 
possible cause is an attempt to promote lock strength (for example a file lock 
from intent to exclusive) when another user holds a shared record lock.
