I haven't yet seen Print Wizard mentioned so I will recommend this as one
of the options to consider.  Like the other products, Print Wizard allows
you to specify overlays (also called underlays or templates) where your
output will print over an image, thus eliminating the need for pre-printed
forms.  But there are two differences between Print Wizard and most of the
others.  First, there are a whole lot more features than just this one
function - it's really closer to the Microsoft Reports Services product in
its ability to serve as a hub between report providers and report
consumers, but with nowhere near the complexity.  Second, the cost is in
the low hundreds of dollars rather than into the thousands.

Print Wizard has had a niche audience in this market for years but I have
only recently begun to really appreciate its simplicity - despite the array
of features - and especially with the reasonable cost.  We've recently
entered into a reseller agreement for Print Wizard, which I believe will do
well in this MV market and outside of this market as well.  I welcome you

Print Wizard does rely on having a Windows system in the mix, but your
output can be provided from any *nix environment to this system in a
variety of ways.
