Hi.  That was exactly the problem.  Mats comments pointed me in the right 
direction.  

What I found was that uv.load had created S999uv.rc files in /etc/init.d/rc2.d, 
/etc/init.d/rc3.d, /etc/init.d/rc4.d and /etc/init.d/rc5.d which were linked to 
/etc/rc.d/init.d/uv.rc.  The uv.rc file actually existed at /etc/init.d/uv.rc.

I simply recreated the symbolic links, then set uv.rc to load (chkconfig uv.rc 
on) and all is now working fine.
