All,

First let me thank you for all your input.  I have learned a lot and now
have a few more tools I can use in the future...even though I really
don't want to have to use them.

Our NA happened to look in the right log file at the right time and it
appears we have a controller on the fritz.  It works sometimes and
doesn't work other times.  It failed on Sunday, but says it's fine now.

I sort of feel better knowing/hoping that this is what has been causing
two weeks worth of stress and file fixing.
