Depends.... If you want to try to put a stealthy record in a file, you it's deliberate.
Most times, it accidental due to a Write who's ID for whatever reason had a value of "" and it will come back to bite you if you don't consider it, case in point I didn't even consider that to be the problem I was having, even though I've been bit before.
