Resetting Ubiquity restored the keyboard focus and fixed that problem, but it did not restore the commands.
Interestingly, I tried removing other add-ons one at a time and discovered that WebNotes was causing the problem. It is a beta product, but still... I wonder how it could conflict so completely with Ubiquity? Thanks for your help, it started me in the right direction.
