I have tried this as well.  In both cases (pushing the number 3 or using the 
arrow keys) the entry does not automatically start orca.  It is activating the 
gnome accessibility; when I do type orca in the run dialog, I don't get the 
 
enable key echo (y/n)
enable braille (y/n)
enable braille monitor (y/n)
routine that ends with "gnome accessibility has been activated you must restart 
for the changes to take effect.  Would you like to restart now?"
I also tried the third entry in the list (same as pushing number 2).  This 
option automatically launches orca as a magnifier.  I haven't tested any other 
options in the list yet because the boot time is so long.  I was considering 
trying to boot into Ubuntu with a different option each day and keep a log of 
what each item in the list actually does (but I still haven't figured out what 
3 is doing).  I'm not sure if that would be helpful for the list to have?
