This is a bug in telephony-service or mission-control.
Only one ofono account should exist at a time, and the ofono-setup script is
probably failing to detect that.

Incoming call
Two notifications are shown, first is as usual, then another without the
action buttons is also shown
Second notifications says unknown caller
The first notification collapses losing the accept decline buttons
First notification times out
Second notification stays until clicked
