Envia esse arquivo em pvt pra mim, irei realizar uns testes, se
funcionar te passo o comando. Pode ser que o arquivo esteja corrompido.
Ou então, faça os testes com outro arquivo, aí você mesmo tira a prova...

Jair, experimente comandar para mp4 assim:
ffmpeg -i arquivo.flv -sameq arquivo.mpg
