I was able to get the upgrade to work using "apt-get dist-upgrade" instead of "update-manager". However, it did not work until I added http://archive.ubuntu.com/ubuntu first in my sources.list. Perhaps there is/was a problem with the mirror http://ubuntu.cs.utah.edu/ubuntu ... although I don't see how that would cause the symptoms described above.
