Recently my sound stopped working.  I have recently done a kernel
upgrade (via the Update Manager), but am not certain that the two are
related.  Here are the symptoms:

I can hear sound from my own microphone
Applications that should be able to generate their own sound (such as Totem) 
show the volume control as disabled.
In the System->Preferences->Sounds there are four test buttons.  Each provides 
an error message.  These messages are:

Sound Events->Sound Playback: audiotestsrc wave=sine freq=512 !
audioconvert ! audioresample ! gconfaudiosink: Could not open resource
for writing.

Music and Movies->Sound Playback: audiotestsrc wave=sine freq=512 !
audioconvert ! audioresample ! gconfaudiosink profile=music: Could not
open resource for writing.

Audio Conferencing->Sound Playback: audiotestsrc wave=sine freq=512 !
audioconvert ! audioresample ! gconfaudiosink profile=chat: Could not
open resource for writing.

Audio Conferencing->Sound Capture: gconfaudiosrc ! audioconvert !
audioresample ! gconfaudiosink profile=chat: Could not open resource for
writing.

Here is what I have tried:
Activated every possible volume control and ensured they are all active with 
volume set to 90%
Searching for .asoundrc (That file does not exist on my system)
Restarting alsa-utils

also-info.sh output URI: http://www.alsa-
project.org/db/?f=081e1d47c330947b3ab054a43a603dfb6087faad

Output of uname -a > uname-a.log, sudo lspci -vvnn > lspci-vvnn.log,
and dpkg -s linux-ubuntu-modules-$(uname -r) >dpkg.log will be attached.
