This is an explicit call to BUG by NFS; while trying to do a radix insert
of an encrypted page, it looks like the page already exists in the NFS
inode page map when it shouldn't. If NFS were to just ignore the issue
rather than bug out, this may work fine. In any event, this is yet another
NFS corner case that we will need to scrutinize.
