It looks like we're getting our wires crossed here. As I said in my previous comment, I no longer think this is a bug. The behaviour is actually not a question of how long my X session has been running. It is simply a function of how long the delay is between middle-press and middle-release. If there is a long delay then the click is not recognised as such, which makes it much harder to accidentally middle- click something when you are using the middle button to scroll. Now that I've got the hang of it I am no longer having any problems.
