I believe that this is also affecting me, though consistently rather
than intermittently. The USB flash disk has 128MB of storage and is
formatted as FAT. It mounted fine with Feisty and Gutsy, but broke with
Hardy. I'm now running Ubuntu 8.10 (Intrepid) Server AMD64 and it
remains broken.

uname -a returns:
Linux CERTIBY-DEV1 2.6.27-7-generic #1 SMP Tue Nov 4 19:33:06 UTC 2008 x86_64
GNU/Linux

When I insert the USB Flash Disk, the following appears in
/var/log/messages:

An unmounted 'USB Memory' disk icon appears in the computer:/// folder.
However, nothing happens when I try to mount it, and no new lines appear
in /var/log/messages.

The contents of /var/log/syslog (when I tried plugging in the USB flash
disk a second time) were:

Once again, the disk could not be mounted, and no new messages appeared
in syslog.
