Thanks for reporting this and helping make Ubuntu better.... I have a couple of 
questions if you don't mind. Are you running 32-bit or 64-bit Ubuntu?
You said that System Sounds work, what about when you launch a game like 
chromium? (this is not a gnome default game you will need to install it: sudo 
apt-get install chromium chromium-data)
Have you tried the other devices that are present by opening gnome-volume 
control and dropping the box down to other discovered devices?

If you run: alsamixer -Dhw is the PCM set at a reasonable level.

Have you also enabled all of the switches in gnome-volume-control and
checked their volume levels?
