Attach your full Xorg.0.log, and the output of the following commands:
* lspci -vvnn
* sudo ddcprobe
* xrandr --verbose
* sudo get-edid | parse-edid # from the read-edid package
If your Xorg.0.log shows "DDC read failed", then perhaps you're seeing a
variant of bug #288807
