Only happens with firefox+smokeyblue.
Doesn't happen with thunderbird, nautilus, gedit.
Doesn't happen with other control themes.

But the finger is strongly pointing towards firefox.

Here's why: when switching between different control themes, firefox
fails to change the width of the scroll bar without document refresh.
(e.g., "thin-ice" scroll bar should be thinner than "human" scroll bar,
but not so in firefox without document refresh). Perhaps smokey-blue
gets confused trying to render its fat bar inside a thin view.

So my initial verdict is: firefox is guilty of not changing the scroll
bar width properly. Smokey-blue is guilty of assuming that it gets its
ideal scroll bar width.
