I have also discovered that if I hold down tablet button 3 or 7, and
press left or right, then the virtual desktop does switch. Which lead me
to try other combinations:

xsetwacom set pad Button3 "core key left"

- button 3 acts as left cursor. I can hold down ctrl+alt and use it to
shift the desktop to the left, or press it to move the text cursor left
in a terminal.

xsetwacom set pad Button3 "core key alt left"

- button 3 generates a D character in a terminal. Holding down ctrl or
alt while pressing it makes no difference. Holding down button 3 and
pressing keyboard left generates one left event on the first press, then
subsequent presses of left generate D characters.

xsetwacom set pad Button3 "core key ctrl left"

- button 3 moves the terminal cursor left one word. Holding down alt and
pressing button 3 generates a D character for each press of button 3.
Holding down button 3 and pressing alt+left shifts the desktop to the
left.

xsetwacom set pad Button3 "core key alt ctrl"

- Works as expected: button 3 has no immediate effect. Holding it down
then pressing left (or right) shifts the desktop.  Holding down left
then pressing button 3 also shifts the desktop.

So it looks like the problem might be something to do with using
modifiers and normal keys in the same button?
