The modules that are in separate packages are modules that are optional, and are not needed for all use cases and by all users. You will notice that the alsa and oss modules are not in separate packages, because they are modules that users will need to make use of pulseaudio through their sound hardware. The jack module is in a separate package because jack is not a standard use case, and only a hand full of users are likely to need that module.
The bluetooth modules fall into the adio hardware category in which users will want to use pulse with bluetooth hardware, but shouldn't have to go to any extra effort to get bluetooth and pulseaudio working toegether. Just because debian may decide to go this route doesn't mean Ubuntu will.
