Un-muting PCM has solved this issue for me, but I don't memorize the status of all my audio sliders (and they're not all at max usually) so this is a really, really frustrating problem that appears to have been going on for FIVE MONTHS. Why isn't someone on the PulseAudio team sorting this out? This is wasting hours of users time! -- New! Sound vanished from speakers.
