I just encountered a similar problem. My machine auto-logins to account A and starts (among other things) Pidgin. I frequently use the fast- user-switch-applet to switch to account B.
Today, after I switched back from B to A and tried to switch from A to B, nothing happened. For an unrelated reason, I shut down Pidgin. When I shut down Pidgin, the fast-user-switch-applet disappeared. I then added the fast-user- switch-applet back to the panel.
