I've changed most settings since I got the USB speakers so I can't be
sure what they were set too originally. If I recall correctly,
originally only movie player and rythmbox would work while VLC, amarok
and flash video through firefox wouldn't.

I solved the problem by following the guide I posted above from the
ubuntu forum. This involves setting all devices in system > preferences
> sound to "PulseAudio Sound Server" except for default mixer track
which is set to the usb speaker. Now whenever an application doesn't
output to the speakers I can use "PulseAudio Device Chooser" (volume
control > playback) to switch the output device from the internal sound
card to the usb speakers (which has a build in sound card).

So basically it's not impossible to fix. But given that these
applications are not installed by default and most people cannot be
expected to even know they exist, most people probably won't solve the
problem on their own. I got lucky by finding the right post.
