Public bug reported:

Upgrade from Ubuntu 8.04 to 8.10 ruined an LVM system that had been
booting with root from LVM for the past three or so years.  Root has
since been reverted back to /dev/sda2 and I've been unable to convince
ubuntu to successfully reinstall LVM root capability (/boot on
/dev/sda1).  Kernel updates in 9.04 alpha now result in crashes claiming
that /etc/udev/rules.d/*-lvm.rules are missing and that

W:copy_exec: Not copying /sbin/lvm to $DESTDIR/sbin/lvm, which is
already a copy of /lib/lvm-200/lvm

Workaround is to copy /etc/udev/rules.d/85-lvm2.rules from a working
8.10 system to /etc/udev/rules.d/85-rules-lvm.rules and manually run
"dpkg --configure -a" with every kernel upgrade
