That's more or less what already happens. The UI in step 3 is the
Launchpad web interface, which also provides a more anonymized way of
additional communication. 4 happens before 3, but that's hardly relevant
for the user experience.
Apport currently doesn't offer you to restart the program after filing
the crash report. It only does that right after the crash ("Report...",
"Restart", "Close"). So is it that what you want?
So the only issue here is that one thing, which would however be an
additional dialog (and as far as I understood you, you want to reduce
the interaction, not extend it).
