I confirm this issue does also occur on ThinkPad T400 bundled with
Vodafone broadband package. The laptop is equipped with Ericsson F3507G
Broadband Module
(http://www.thinkwiki.org/wiki/Ericsson_F3507g_Mobile_Broadband_Module).

Simply, I can connect manually, by configuring and calling wvdial,
however Network Manager can not "see" such connection and does not setup
itself to online mode. So, Firefox states it's offline (interesting
thing is that Thunderbird is online without problems).

Unfortunately, I'm unable to offer any help in fixing this myself.
However, if there is a hacker in London area, I'm willing to offer my
ThinkPad T400 + good dinner & drinks for testing  and fixing this issue.
If there is anyone, feel free to drop me a message.
