This is one of those things where, in retrospect, I wonder why it ever
*did* work...  I have a "ghetto KVM" setup here, using a monitor with
dual inputs and manually swapping the single USB cable for a "wireless
desktop" keyboard and mouse package between two machines.  The one in
question is running 64-bit Ubuntu 9.04.

Oddly enough, this worked well enough until today, March 15, '09, when I
accepted the latest software updates.  Now it seems that removing the
USB input device(s) doesn't hurt anything, but the act of reconnecting
them kills the X server.

The installed versions of likely culprits are:
Xorg "1:7.4~5ubuntu15",
HAL "0.5.12~rc1+git20090204-0ubuntu3",
gdm "2.20.9-0ubuntu1"

I can't seem to find the complaint in Xorg's logs right now; is gdm to
blame?

... I also can't be completely sure if this is actually being triggered
by a change in software or it's just that the system booted in a
different state (keyboard/mouse not connected and not taken over by
'legacy' emulation) than usual, though given that I've replugged dozens
of times in previous sessions and don't think I've ever met a BIOS
that'd re-acquire USB devices for "legacy" service, perhaps that's
unlikely to be the issue.

Just curious for any insight others may have; I'm not sure this wouldn't
also be a problem with actual USB KVMs of sufficiently cheap
construction.
