Indeed, I think this has been fixed by our various changes to avoid
partitioning race conditions, including the bug Timo refers to but also
other work to add sequence points between racy LVM operations. I won't
mark this as a duplicate since you could have been running into any of
those.
Chris, please reopen this if it happens to you again on Jaunty Alpha 6
or newer. Anyone else, please file a new bug if something similar
happens to you on Jaunty Alpha 6 or newer.
