Here's the promised log-files and photographs to make the issue easier
to understand.

Note: the xserver doesn't fallback into fail-safe mode in this scenario,
it is only a case that monitors show no output.

Here's the tests I did, after which I took a photograph and copied the
current /etc/X11/xorg.conf and /var/log/Xorg.0.log

1. Default xorg.conf, no external monitor connected (laptop display shows 
800x600 centered in 1024x768 display)
2. Default xorg.conf, external monitor connected (both displays blank)
3. xorg.conf with manual monitor settings, external monitor connected (both 
displays display at 1024x768)
