I concur with Michael that it looks like a hardware problem: either a temporary problem with the CD, or possibly with the hard disk. I disagree with Michael that it makes sense to copy files to the hard disk first: that slows down the upgrade, and introduces problems related to available disk space.
Other people don't seem to have had this problem, and it happened a long time ago.
