Confirming this based on the behavior of ubuntu live cd image 20090324;
it occurs only when selecting the install option from the syslinux boot
menu, not when running the live image and starting ubiquity off of the
desktop. It appears to be doing exactly what Colin suggested, which is
that the dialog box is appearing behind ubiquity. Doing alt-tab shows
two active apps (at times); however, attempts to bring the dialog to the
foreground don't work. This likely doesn't occur in the live cd session
install because a full window manager is running,
