I see this also in OpenOffice.org 3.0.1.
Download and open this word document:

Scroll down and click on a checkbox or something. Save it and close it,
then re-open it and scroll down.
Instead of checkboxes and text fields you'll see FORMCHECKBOX and
FORMTEXT.
