There should be: (1) 12px spacing between the top of the text and the bottom of the title bar (2) 12px spacing between the top edge of the icon and the bottom of the title bar (3) 12px spacing between the left edge of the icon and the left edge of the window (4) no separator in the window at all (5) 24px spacing between the bottom of the text and the top of the buttons (6) a distinction between the Cancel and OK buttons, which should have the same width as each other, and any other buttons, which should have unrelated widths.
