For reproducing this bug, form IT-203 is an even better case:
http://www.tax.state.ny.us/pdf/2008/fillin/inc/it203_2008_fill_in.pdf

Here, after selecting option 1 from the large (A) Filing Status check
box fields in acrobat, saving the PDF file, and opening with evince
changed the selection from option 1 to option 5, even though it stills
shows correctly in acrobat.
