In the past we had slightly different behaviour in setuptools if
installed with --single-version-externally-managed or not. I didn't
check for this in recent versions. To break setuptools out of virtualenv
we would need to have the setuptools source available at virtualenv
runtime, so a setuptools-source package would be needed (built by the
setuptools deb package).
