Booting without USB attached didn't fix either problem. In the end, I used rsync to clone Ubuntu from my old hard drive onto my new hard drive, and after I rebooted, all of the problems went away. Suspend/resume started working again, X stopped crashing when I changed resolutions, and I'm not getting these phantom READ CAPACITY problems any more, either.
