are you using auto login?

My guts feeling tells me that this happens because nm-applet is running
while NetworkManager isnt fully up yet.

I wonder if this will trigger this issue for you:

I. variant A:

 1. stop NM: sudo /etc/init.d/NetworkManager stop

 (wait a bit)

 2. kill nm-applet: killall nm-applet

 3. start nm-applet: nm-applet

 4. start NM: sudo /etc/init.d/NetworkManager start

II. Variant B
 If this doesn't trigger it, please also try to flip the order of 3. and 4.

III. Variant C
 after 2. also sudo killall nm-system-settings and try variant A and B ;).
