I must say that I'm a bit surprised that you didn't take the effort to
reproduce this problem yourself. Especially since fillable PDF forms
are used for important things such as filing taxes. Also, I'd raise
the priority because of this reason.

In any case, I have attached a fillable PDF that reproduces the
problem. This file was created using Adobe Reader 8, v8.1.3
10/07/2008. I simply opened the empy form, selected the following
options: page 1: Filing Status "1. Single", page 2: (line 33)
"Standard Deduction", page 4: (line 72) "Refund" and (line 72b)
"Checking".

Reopening the file with Adobe Reader shows the correct selections.
Reopening it with Evince shows: page 1: "5. Qualifying widower", page
2: "Itemized Deduction", page 4: "Owe" and "Savings".

I hope this helps to pinpoint the bug,
