I think I've fixed this, though it needs to be confirmed. This is a hardware issue with the Creative SB Audigy 2 NX, I think. In short, going into the BIOS and setting the onboard USB controller mode to USB V1.1 sorted it (before, it was set to V2.0 and V1.1).
