I think you might have used the wrong template. That seems like a error
reporting template specific to a single package. Please give more
details about what you are doing and what programs were right before the
crash. Also, are you using the standard Ubuntu kernel or are you using a
modified kernel? Also try starting Ubuntu in a non-graphical mode (Click
on "Settings" in the login window) and see if that happens.
