Same situation here. I believe in my case, this is triggered by turning effects On (System > Preferences > Visual Effects), and subsequently turning Visual Effects back off. While 'On', ALT key combos work fine. Then when turned off, ALT is ignored. Pressing ALT-TAB twice when focus is on a terminal yields:
