1. What I did:
I reported an issue about openoffice.org here on bugs.launchpad.
Nearly at the bottom it said:

I pressed the "Browse..." button and selected a file.
The I pressed the "Submit bug report" button.
2. What happened:
- The bug-tracker was created, but the file was NOT attached.
- 3. This is what I expected: That the file would be attached. when creating
the tracker.
