For posterity and future web surfers: This error message can also be
caused by failing hardware. (This error crops up in bug reports that are
never resolved because the poster does not return to announce "actually,
my USB died.") In particular,
http://www.fixya.com/support/t1964855-ibm_thinkpad_t43_freezes_until_pressure
will cause this error to be thrown. In laptops, it is often due to the
motherboard flexing over time and solder connects breaking loose, and
T43s are not the only victims; many different kinds of laptops display
similar problems. However, in this case all your USB devices would fail,
not just a single device, and pressure or flexing may not solve the
problem.
