Public bug reported:

If you enable focus-follows-mouse, and use the alt-tab function,
sometimes focus follows the mouse after the alt-tab dialog disappears.
The conditions don't seem entirely deterministic, but here is a tip: the
problem doesn't occur if the mouse cursor is NOT WITHIN the alt-tab
dialog, nor will the problem occur if the mouse cursor is WITHIN AN
WINDOW ICON/PREVIEW within the alt-tab dialog. (What you should take
away from that: make sure the mouse cursor is located near the edge of
the alt-tab dialog.

I tried recording a video with Istanbul to show this problem, but
Istanbul doesn't even appear to work either! I am submitting a bug for
that, too, as I have some gstreamer experience.
