When I had a problem with my USB HUB I used "mseg|grep usb" and the error shown was "62". That seemed to be an error involved with cabling.
My USB HUB has 4 USB ports. When I use two ports, I had to have them side by side at one end. Then it worked. Any empty ports in between and it failed. It seems that Ubuntu checks the USB ports from a hub from one side, knowing how many attachments exist, but when one USB port is empty it tries to make the connection anyway and stops looking further if it fails.
