When I had a problem with my USB HUB I used "mseg|grep usb" 
and the error shown was "62". That seemed to be an error involved with cabling.

My USB HUB has 4 USB ports. When I use two ports, I had to have them side by 
side at one end. Then it worked. Any empty ports in between and it failed.
It seems that Ubuntu checks the USB ports from a hub from one side, knowing how 
many attachments exist, but when one USB port is empty it tries to make the 
connection anyway and stops looking further if it fails.

Perhaps the driver software needs to be improved to ensure it looks at
all USB ports, even if some are empty, when there is no communication.
