Indeed it would be great to nail down a reproducible set of steps.
Looking at the linux-ext4 mailing list, it seems others have seen this
same sort of issue and resolved it with an fsck:

I do also notice you mention running Jaunty which uses a 2.6.28 based
kernel.  I'd suggest maybe trying the latest 2.6.31 based kernel for
Karmic, it's had quite a few ext4 related patches applied.  ISO CD
images of the latest Karmic Alpha4 are available at
http://cdimage.ubuntu.com/releases/karmic/ .  It would be good to know
if this reappears with the newer kernel.
