I downgraded to Ubuntu 8.10, and my large block size transfers (between 2MB and 3MB) are working correctly now.
Based on my experience, I believe the qla drivers may be broken in 9.4, for block sizes larger than 1kB. It looks like none of the developers are following this bug report, maybe because the bug report is waiting on input from the original poster. If I have time, I will submit a new bug request in regards to the block size issue. If anyone wants to work on this issue, I should be able to provide additional information to help debug the problem, if you can talk me through it.
